If you are submitting your success story and pictures by mail, you must have your parent or legal guardian print and sign the media release form here. If you are uploading pictures, be sure to have a parent or legal guardian complete the Online media release form. Signed media release forms will permit the use of photos for every person who appears in submitted pictures.

On behalf of my minor child, I state and agree: I am my child’s parent or other legal guardian with authority to sign this Consent on my child’s behalf. I consent to my child’s attached submission, which includes my child’s name, address, story, pledge and photos. My child’s submission is true and is the original work of my child. In consideration of my child participating in Take a Stand, I assign to Sunkist in perpetuity all ownership of any all other rights to my child’s submission, including the story, pledge, and the photographs of my child. In further consideration of my child participating in Take a Stand, I consent to Sunkist’s unlimited use of my child’s first name, age, city and State of residence, story, pledge, and photographs for all purposes, including advertising and other promotion in all media, related to Take a Stand. Sunkist shall have no obligation to compensate my child or me with money or other tangible item. I release Sunkist in connection with all liability related to my child’s submission and related to any acts or omissions of my child in connection with the submission and Take a Stand.
